Background: Medial compartment osteoarthritis in young active patients presents a unique challenge with regard to joint preservation. Interventions, including cartilage restoration procedures, the setting of high tibial osteotomy (HTO) have potential obviate or delay replacement if performed degree accuracy and avoidance complications. Indications: The procedure is indicated less than 65 years isolated medial knee arthrosis, good range motion, no ligamentous instability. Technique Description: We present our technique for valgus producing opening wedge HTO patient-specific instrumentation implant concomitant osteochondral allograft femoral condyle microfracture. Results: goal this intervention provide minimally painful durable (>10 year) outcome while minimizing risk perioperative complications iatrogenic fracture nonunion. Discussion/Conclusion: High repair techniques can lead satisfaction return sport rates appropriately selected patients.
